Completing the birth notification.
- U.A.E citizen
- Discharge card issued by the ward on discharge
- Identification: nationality paper + passport for father and mother (original + copy)
- Incase if the father and mother not mentioned in the nationality paper or they r different nationality – they ask for marriage certificate
- If the father or mother without nationality they have to go to the office and sign the special form (personal affidavit)
- Non U.A.E citizen
- Discharge card issued by the ward
- Photocopy of the valid passport of the father and mother with valid residency visas
- Photocopy of authenticated marriage certificate if the father is not indicated in the mother’s passport.
Fees
- 50 Dirhams for local
- 100 Dh for non local
