Definition A Blank Certificate Form issued by DOH&MS to Government Organizations on Credit basis to facilitate the Medical Check up of their Employees in order to complete the Immigration requirements to issue a resident Visa. Procedure The following are required in order to obtain this service: Valid Health Card from Department of Health and Medical Services, Government of Dubai. Medical Fitness Certificate that can be purchased from Department of Health, Health Card Section which is situated in Rashid Hospital, or can be purchased from Medical Fitness Centres in Al Maktoum Health Centre or Jebel Ali Clinic Medical Fitness Certificate must be typed in Arabic language. Rules & regulations Rules & regulations The Government Organization apply for Credit Facility with DOH&MS DOH&MS receives the letter The management approves the request & instructs the concerned to open An Account DOH&MS intimates the Organization of its acceptance Whenever the Organization needs Medical Certificate Book (Set of 50 Blank Certificates), their representatives approach DOH&MS with a letter specifying the requirement & quantity of the certificate. DOH&MS issues the Book DOH&MS at the End of the Month calculates the total numbers of books issued & their value, prepares an Invoice & forwards it to the concerned Government Organization for payment DOH&MS, within a month, receives a Bank Transfer, Cheque or Cash from the beneficiary.
