The department of Health & Medical Services provides renewal of health card for corporate employees to facilitate the procedures, save time& reduce cost by providing the services electronically.
Procedures
- Collection of valid and basic patient’s information.
- Registration of the patient’s information in the computer system.
- Issuing Health Cards Nos.
Rules & Regulations
- The Government Organization applies for Credit Facility of Health Card Corporate with DOHMS
- DOHMS receives the Application
- Management approves the Application & instructs the concerned to prepare Accounts Opening Approval Form
- DOHMS intimates the Organization of its acceptance & requests them to fill up
Corporate Registration Form.
- The Organization furnishes DOHMS with a floppy disk contains their staff Health Cards Numbers.
- DOH&MS feeds all Health Card Numbers in the SAM Application System.
- The organization may start the renewal process.
- DOH&MS at the End of the Month generates a Financial Reports of all renewals made by the Organization, prepares an Invoice & forwards it to the concerned Government Organization for payment
- DOH&MS, within a month receives a Bank Transfer, Cheque or Cash from the beneficiary.
